Medicare requires the scope of appointment form (permission to talk about plan details) at least 48 hours prior to an appointment. In the link below, you'll fill out your demographics, doctors, medications and that scope of appointment form. Be sure to initial ALL the boxes beside the plan types. Once signed, click submit. This information keeps me compliant with regulations and will save us a great deal of time during your strategic planning session.
