For clients requesting an in person Physical Therapy evaluation. Please note that your appointment request WILL NOT be confirmed until you submit payment and complete your registration and intake paper work. You will be directed to a link to complete this process. Please complete this ASAP to avoid your request from being booked by another client.

First Time Client Consultation
Service Description:
In this initial 15 minute phone or video consultation, we will have a conversation about any problems or issues, desired goals, and ways they can be affected. Please make sure to include in the note section your health or wellness concerns.
If you choose to continue working together after this initial consult, prices are $80/30 minutes.
Stipulations as follows:
Due to the popularity of this service, there is a 5 minute grace period before you are marked a no show.
One per client.
Please schedule a 30 minute consult after this should we choose to move forward.
