Year-End Performance Evaluation
As we approach the end of the year, it’s time for our annual performance evaluation. This is an important opportunity to assess your progress, reflect on your achievements, and identify areas for growth.
To ensure a comprehensive evaluation, the following documents are required:
- Customer Satisfaction Feedback Forms (CSFF): Any feedback that commends your performance from clients or customers.
- Incident Reports (if applicable): Documentation of any workplace incidents that may have occurred.
- Duly Filled out Performance Evaluation (PE) Template: This form is part of your evaluation and needs to be filled out honestly.
- Manager's Feedback on PE Template: This section will be completed by your manager after reviewing your submission.
Instructions:
- In your Personal Evaluation folder under your 201 files, please [Make a Copy] of the most recent Performance Evaluation template in the same folder.
- Remove all previous information from the copied form, except for your personal details, to create a blank template.
- Complete the form and forward the link to your manager when done.
- Please provide honest and thorough feedback. This evaluation is a professional assessment and should be completed thoughtfully.
